Parent/Guardian Consent & Acknowledgment of Risk I, the undersigned parent or legal guardian, hereby give permission for my child to attend and participate in yoga classes, workshops, and related activities at Life Yoga (20217 Hall Rd, Macomb, MI 48044).
I understand that yoga involves physical movement and that participation may expose my child to potential risk of injury, even serious injury. I acknowledge and accept these inherent risks and affirm that my child is physically fit and has no medical condition that would prevent safe participation in yoga or related movement activities. It is my responsibility to consult with a physician prior to participation if I have any concerns regarding my child’s health.
Assumption of Risk & Waiver of Liability In consideration of my child being allowed to participate in Life Yoga classes or programs, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, that my child may incur as a result of participation.
I hereby release, waive, and discharge Life Yoga, its owners, teachers, staff, and landlords from any and all liability, claims, or demands for injuries, damages, or losses that my child may experience or sustain in connection with participation in Life Yoga activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.
